Additional Information
In addition to providing therapy and professional training, I conduct mental health evaluations and substance abuse evaluations for both adults and juveniles.
Treatment evaluations typically take 1.5 to 3 hours to complete and are designed to determine whether or not an individual meets diagnostic criteria for a mental health or substance abuse disorder and what, if any, treatment recommendations would be suitable.
As a Forensic Mental Health Evaluator, I also conduct forensic mental health and substance abuse evaluations used in criminal and civil court proceedings.
As a Master's Level Certified Addictions Professional (MCAP), I am also qualified to conduct addiction/substance abuse evaluations for individuals who have violated a U.S. Dept. of Transportation (D.O.T.) rule regarding substance use.

Individual Session
$150 for a 52-minute session.
Substance Abuse Evaluations
$375 for clinical evaluations with a typed summary letter
$550 for clinical evaluation with full typed report
$750 for forensic evaluation (full report written to the a court or attorney providing an expert opinion for a legal proceeding)*
Mental Health Evaluations
$450 for clinical evaluation with a typed summary letter
$750 for clinical evaluation with a full typed report
$950 for a forensic evaluation*
* In many cases, extensive medical records may need to be reviewed, or interviews will need to be conducted with people other than the client. Such a activities are billed at an additional rate of $150 per hour.
Professional Training & Consultation Rates
Rates very depending on several factors. Please call me at (305) 812- 6018 for a free phone consultation and quote. Expert testimony in a legal proceeding is billed at $250 per hour including travel time with a 3-hour minimum.

In addition to private pay, I am contracted with the following insurance panels and third party payers:

If you do wish to use insurance but to not see your plan listed above, you may be able to see me through your out-of-network benefits.
Payment
Cash, check and all major credit cards accepted for payment.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
